One day a blind man goes to a Pharmacist for medication.
After the Medication he asks the Pharmacist about his bill. "I am sorry, I cannot accept money from you I am doing community service for the handicapped this week." The blind man is pleased and leaves the shop.
The next morning when the Pharmacist goes to open his shop, there is a thank you card and a dozen roses waiting at his door.
Later a man in wheelchair comes in for Medication and he also goes to pay the Pharmacist, and the Pharmacist replies: "I am sorry I cannot accept money from you. I am doing community service for the handicapped this week." The wheelchair man is happy and leaves the drug store.
The next morning the Pharmacist goes to open his shop and there is a thank you card and a box of a dozen Plantain waiting at his door.
A deaf man comes for medication and when he asks the Pharmacist what he owes, the pharmacist wrote on paper: "I am sorry I cannot accept money from you. I am doing community service for the handicapped this week. The deaf man is very happy and leaves.
The next morning when the Pharmacist goes to open his shop, there are a dozen deaf people waiting at his door.

If you are employed by the Ministry of Health to work at one of its hospitals, must one go and learn sign language because deaf people may come to the facility? In civilized societies, interpreters are assigned. Why not suggest it should be part of the medical training? Probably all doctors in New York know sign language. My wife delivered all our children in hospitals. Maybe, we were not sophisticated because we did not have relatives in New York, and could not afford private clinics.

Disabled-friendly access to facilities/service points need to be available as well.

I disagree because Chief Complain of a patient is only an explanation of the HPI: History of present illness. What determines a diagnosis is a combination of Review of systems, examination, vitial statistics medical history, etc. Doctor reviews all these, and sometimes including blood work or imaging in order to reach an impression/diagnosis or assessment.
Therefore what you complain to the doctor for the most part may not be the doctor's impression (diagnosis)

Today, and years past you could help in this exercise for a living. But providence decides better that, you had not even a day in the classroom, than we taught you at home. A bracelet on deaf/blind/epileptic/asthmatic/those who dont want blood etc would be a good way to solve problem
