Committee To Probe Fire Outbreak At Refinery Set Up

Accra, Aug. 15, - The Ministry of Mines and Energy has set up a five-member committee to investigate the cause of the July 18 accident at the Tema Oil Refinery (TOR) which claimed two lives and damaged equipment worth millions of dollars. A statement issued by the Ministry in Accra today said the Committee, which will be meeting at TOR, has two weeks to present its report. Members are Mr J. C. K. Fynn of Ergs Engineering, chairman, Mr H. M. Minski of GNPC, Mr I. C. Tagoe of the Ministry, Mr C. O. Tetteh, formerly of TOR, and Major T. A. Darteh, also of the Ministry, secretary. The Committee will, among others, examine and analyse significant causes of the accident, especially operational lapses and responses and emergency actions taken by TOR to determine their adequacy or otherwise. It will also examine maintenance and training procedures and records related to safety and prevention of accidents, determine the total cost of the accident and measures to finance the rehabilitation and propose measures to forestall future occurrences. The statement invited members of the public wishing to contribute to submit written memoranda to the Committee.
