But there is nothing mysterious about accidents happing at places of work,even in more advance Countries than ours,it do happens.But to prevent it happening again,there should be a seriuos investigation, to know why it happened in the first place.

There is a reason why machine are properly lockout whenever work has to be done on them. This is to prevent any UNEXPECTED RELEASE OF ENERGY OR STARTUP!
Just because the switch is on the off position doesn't mean it can't be accidentally started. You have to perform lockout on the machine first using any applicable lockout device and a lock with a key.
It is the only of the company's responsibility to ensure the worker is properly trained and every hazard inherent in the job is made known to the worker in addition to providing the appropriate personal protective equipment for the job.
If the owner failed in this regard, then s/he is culpable for this workplace fatality.
May his soul RIP nonetheless!

Those of us who worked at Valco know that we have what is called lock-out of machines before work is done. In other words the machine is switched off and the switch is padlocked. It is only the worker who has the key to the padlock. If there are several workers in the mixer, each one of them will have his own key. No two keys are the same.The padlocks are removed by the workers one after the other as they leave the mixer. It is only after the last padlock has been removed before the switch can be put on.
If you forget and leave the padlock on and go home the supervisor will have to trace you to your home and bring you to site to remove it.
Management of this company should be held responsible for this unfortunate incident. My condolences to the bereaved family.
There was a similar incident last year when an ECG worker was electrocuted in a similar situation. This time the electrician went to respond to power failure in the Kwabenya area and failed to isolate and lock the power out with a padlock.
Health and safety is an issue in Ghana . Our MPs should enact laws to hold employers accountable for the health and safety of their employees.
This lock-out system is what is practised everywhere else in the world. Ghana is always in a catch-up mode as usual.
OUR LEADERS SHOULD ACT.
NOBODY WANTS TO HAVE HIS RELATIVES KILLED AT WORK FOR THINGS LIKE THIS.

Machine inspectorate division reading please expedite immediate action by stopping operations of TEMA DEVTRACO LTD they will kill more guys shortly .
The project Manager MUST be prosecuted or put behind bars pending investigations and jail later .No AMOUNT of compensation can bring this guy back.

If there were proper and effective health and safety rules in practice, the deceased would certainly not have climbed into a potentially dangerous machine to clean it. It springs to mind that the company knew that workers were taking unwise risks but did nothing about them. The company should be held responsible for the worker's untimely death. I wouldn't agree if someone were to say that the deceased was reckless in climbing into the machine. The fact is that if he was trained to do a potentially dangerous job but was reckless and negligent, then one could say that he contributed to his demise. The manner of his death doesn't indicate that safety measures were enshrined in the company's rules. The company should be brought to book.

I am upset by the graphic photo. It is not right on the part of Peace FM to publish the picture. In today's world, anything published on the internet stays on forever. This is not the imagine his family needs to see of him. His children need not see this, either is the general public. All Peace FM cares about his how much traffic it's going to generate on their site. Also, i went to visit Devtraco site at community 25 today. I went there to look around and talk to a few workers just to know the kind of operation their running. I was told by one of their workers (whose name i am not going to mention) that when ever they introduce a new machine, a representative from the country where the machine is going from is sent to Ghana to teach key workers the proper use and safety measures of the machine. Also i realized that not all the workers were in safety gear. I asked the ones who didn't have the hats, gloves etc on. The stated the company has provided them these items but have refused to use them because of heat( hot sun) and other factors. Some also mentioned that they never thought it was important to use all the safety equipment allocated to them. It is extremely sad that something like this happened to one of your own. RIP. May God be with his family.

6. Tool box meeting conducted and signed off by worker?
7.When was the last time TEMA DEVTRACO LTD was AUDITED IN TERMS OF SAFETY.
8.Is the operating procedures and associated hazards known ?
9.Any emergency stoppers observed on the mixer and task supervised ?
10.Evidence of employees entry induction and medical certificate of fitness of all employee available for investigation purposes ?
