The problems Ghana is facing regarding congestion at our hospital will be a thing of the past with the family doctor concept. I say they should go further by introducing a triage system. A sick person does not always have to see a doctor in the first instance especially given that we have few doctors in Ghana. In the UK, they have the General Practitioner system where, apart from emergencies, the family doctors running the General practices with their team of nurses and other allied health professionals are the first point of call for patients. The patient is then referred to the acute sector (hospital) or a specialist health service provider if the family doctors assesses and feels there is a need for a referral.
so before a patient gets to the hospital, there would have been some pre assessment and possibly first line treatment before they get to the hospital if at all. This has many advantages:
The patients' conditions are detected much sooner and treatment given before they deteriorate to a point where prognosis becomes bad.
Patients are also screened by their family doctors and the necessary advisory and preventative measures are given before their symptoms degenerate into a debilitating condition. This prevents the kind of problem we have in Ghana where the only time one knows they are hypertensive is when they present at the hospital with a stroke.
Prevention and early treatment is cheap and life saving.
Family doctors will filter patients through the triage systems and prevent our hospital from choking with too many unnecessary sick people.
There are so many people walking about with diabetes in Ghana without even knowing it until they collapse and are rushed to the hospital. That need not be the case. Family doctors can screen for such conditions, detecting early warning signs, and preventing the government from spending loads of money to treat victims when their diabetes finally lands them in hospital.
In the UK, the General practitioners are fund holders; they provide their services to the department of health saving the uk department of health a lot of money and enhancing gains in efficiency.
Having such a system in Ghana will prevent doctors from going abroad. Let them stay in Ghana and run private Doctor led clinics and sell their services to the Ministry of Health. Wonderful idea.
